Who Qualifies in Ottawa County?
Person receiving care:
- Has active Michigan Medicaid
- Needs help with daily activities (bathing, dressing, mobility, toileting, eating)
- Lives in a private home — not a nursing facility or assisted living
Family caregiver:
- Age 18 or older
- Not the spouse of the recipient
- Passes MDHHS background check through CHAMPS

Caregivers enrolled through HHN earn $18 per hour as of January 1, 2026.
Do families near Kent County use the Ottawa office?
Ottawa County is served by its own MDHHS office. If you live in Ottawa County (including Hudsonville, Jenison, or Grandville), you go through Ottawa County MDHHS, not Kent County.
