Who Qualifies in Kent County?

- Has active Michigan Medicaid
- Needs help with daily activities (bathing, dressing, mobility, toileting, eating)
- Lives in a private home — not a nursing facility or assisted living
Family caregiver:
- Age 18 or older
- Not the spouse of the recipient
- Passes MDHHS background check through CHAMPS

Caregivers enrolled through HHN earn $18 per hour as of January 1, 2026.
Can someone in Rockford or Lowell use this program?
Yes. The Kent County MDHHS office serves all Kent County communities — rural and suburban families are eligible the same as Grand Rapids residents.

For the initial assessment, an MDHHS caseworker comes to your home. You'll need to call the office to schedule that visit.
